The issue is that if the 60V main MOSFET breaks down (avalanches) it can take the switcher controller with it. The MOSFET seems to take it just fine, but the controller is a little sensitive.

A few large parallel caps will help a lot, a few large TVS devices will also help. But so much depends on the actual setup it's hard for me to determine how much capacitance/TVS value will work well.
